PATIENT PORTAL ACCESS
Our secure, HIPAA-compliant patient portal is automatically activated for any patient with an appointment scheduled and an email address on file.  When activated, an email will be sent with your temporary password to login. Your username will always be your email address. If you have not provided our office with an email address and wish to have access to your portal account, please contact our staff for assistance.

 

NEW PATIENTS
To become a new patient, please call our office to schedule an appointment after which time we will email you additional information on how to complete the necessary paperwork online.
